ADRC extended partners are dedicated to increasing consumer access to services, diverting persons with disabilities and older adults from unnecessary institutionalization, and supporting a streamlined “No Wrong Door” referral process for the four core ADRC services:
- Enhanced Information and Assistance
- Short-Term Service Coordination
- Person-Centered Options Counseling
- Transition Support.
